Fundraising Agreement Terms and Conditions
Alzheimer’s Australia (Qld) Limited reserves the right to withdraw approval for this fundraising activity at any time if it believes any aspect of the proposed fundraising activity no longer fits within its fundraising guidelines. Any fundraising activity undertaken is not an activity of Alzheimer’s Australia (Qld) Limited and therefore all responsibility, including public liability, lies with the Activity Organiser who is raising funds to donate to Alzheimer’s Australia (Qld) Limited. Alzheimer’s Australia (Qld) Limited is not responsible for taking on a coordinating role and cannot guarantee to help solicit prizes, organise publicity, provide goods or services or a representative on the day, to assist the Activity Organiser in the running of the fundraising activity. However, where possible, support and advice will be offered. For your fundraising activity to be approved you will need to complete and sign this Community Fun-Raising Application form. Before you may begin to fundraise you must receive an “Authority to Fundraise” letter from Alzheimer’s Australia (Qld) Limited. The Activity Organiser must not approach any organisation asking for prizes and /or sponsorship without prior approval from Alzheimer’s Australia (Qld) Limited. This is important as Alzheimer’s Australia (Qld) Limited has its own sponsorship agreements in place and the organisation you approach may already support Alzheimer’s Australia (Qld) Limited and we do not want them approached on our behalf more than once. You are responsible to ensure your activity meets any requirements of relevant laws and regulations. Any use of the Alzheimer’s Australia (Qld) Limited name must be first approved by Alzheimer’s Australia (Qld) Limited. All printed materials including media releases must be approved by Alzheimer’s Australia (Qld) Limited before printing and circulating. Alzheimer’s Australia (Qld) Limited will not send out receipt books. The Activity Organiser may submit a list of names and donation amounts via the Donation Details form and Alzheimer’s Australia (Qld) Limited will send completed individual receipts to the Activity Organiser to distribute. Donations of $2 or more are tax deductible. The following are not tax deductible: ticket purchases (raffle, entry to an event etc), donations of goods and services, or auction items. The Activity Organiser is responsible for all expenses relating to the activity. Money raised, your payment slip and the Donation Details form must be returned to Alzheimer’s Australia (Qld) Limited within four weeks of this fundraising activity.
